Request Student Special Education Records

The information presented on a student's Individual Education Plan including evaluations are considered confidential and protected by federal law.   A request for records can be done by phone, in person or by email.  If you want someone else to be given a copy (attorney, friend, advocate) we must have your permission in WRITING with your signature to release records. The following information is needed to to complete your request form:

  1. First & Last Name of Yourself/Child

  2. Date of Birth

  3. Current Address

  4. Contact Number

  5. Last School attended or graduated from and graduation year (if applicable)

Student over the age of 18 are considered adults and must request records themselves. A parent is NOT allowed to request records or pick up records unless the adult student gives consent in writing.

  • Records will be available within 5 business days of the request date.

  • Please schedule an appointment for pickup when the request is complete. 

  • Upon pickup, a Photo ID is Required as proof of identity. 

(Please note that records MUST be picked up in person and signed for. They cannot be mailed, emailed or faxed.)

The first copy is ALWAYS free. Additional copies may be 10 cents per page.
