  • Parent/Guardian Involvement Survey

We are making an effort to find out how every student gets home each day. We understand that you may use MULTIPLE methods at dismissal time, but we would like for you to record your PRIMARY method (the one you use the most). We are including a space at the bottom of this survey for your to specify any regular changes in your routine. If you have MORE THAN ONE student at RHES, please submit a separate one of these surveys for EACH student. This is important for our planning and for the safety of our students. The information you share will only be used by our staff and will be kept confidential.

Please take a few minutes to fill out this survey
